​Dear Parent/Guardian,

Thank you for your interest in KIDDO for Term 3, 2026. Please complete the following enrolment form and payment if you wish your child to participate. Before completing this form, please read the following conditions of enrolment:

CONDITIONS OF ENROLMENT
Enrolments will NOT be accepted without payment. You will be directed to make your payment online upon completion of the enrolment form. Enrolment forms can only be submitted online. Please ensure you have read and understood the refund and missed session policy.

COSTS
The 8-week KIDDO program fee is $130 per participant to be paid on enrolment

PROGRAM DATES
Term 3 Skills: Catching and Jumping

Mount Hawthorn PS: Tuesdays 7:50 am - 8:35 am
Sessions Start: Week 3 of Term 3 (Tuesday 4th August)
Parent Week: Week 9 of Term 3 (Tuesday 15th September)
Sessions Finish: Week 10 of Term 3 (Tuesday 22nd September)

REFUND & MISSED SESSION POLICY
No make-up sessions are available for missed or cancelled sessions. No refunds will be given for missed sessions. No refunds will be given if you change your mind about your child participating in the program. Sessions may be cancelled in extreme weather conditions. All refund requests must be in writing.

COMMITMENT TO RESEARCH
KIDDO is committed to ongoing research, and on occasion, you and/or your child may be invited to participate. Acceptance of the invitation to participate in our ongoing work is completely voluntary and has no bearing on program participation.
